Requesting a Vote-by-Mail Ballot

A vote-by-mail ballot covers all elections throughout the calendar year. Once you submit a request for a vote-by-mail ballot, you will not have to renew it until the following year. 

To request a Florida vote-by-mail ballot, you can: 

 

To request a Florida vote-by-mail ballot, you must include the following information: 

  • Name 
  • Date of birth 
  • Legal Residential Address (If the request is to mail the ballot to an address other than the one on file, the request must be a signed writing) 
  • Florida driver license or identification card number, or last four digits of social security number 
  • Signature (If request is written)
