Requesting a Vote-by-Mail Ballot
A vote-by-mail ballot covers all elections throughout the calendar year. Once you submit a request for a vote-by-mail ballot, you will not have to renew it until the following year.
To request a Florida vote-by-mail ballot, you can:
- Write a signed letter requesting a vote-by-mail ballot to Supervisor of Elections and send via mail, fax, or scanned attachment to an email
- Request a ballot in-person at your local Supervisor of Elections' office
- Request a ballot over the phone by calling your local Supervisor of Elections’ office
To request a Florida vote-by-mail ballot, you must include the following information:
- Name
- Date of birth
- Legal Residential Address (If the request is to mail the ballot to an address other than the one on file, the request must be a signed writing)
- Florida driver license or identification card number, or last four digits of social security number
- Signature (If request is written)
